Drugs Use.

Oral contraception. The mode of action includes the inhibition of ovulation by suppression of the mid-cycle surge of luteinising hormone, the inspissation of cervical mucus so as to constitute a barrier to sperm, and the rendering of the endometrium unreceptive to implantation.

Dosage and method of administration

First treatment cycle: 1 tablet daily for 21 days, starting on the first day of the menstrual cycle. Contraceptive protection begins immediately.

Contraindications

Combined oral contraceptives (COCs) should not be used in the presence of any of the conditions listed below. Should any of the conditions appear for the first time during COC use, the product should be stopped immediately.

• Presence or a history of venous or arterial thrombotic/thromboembolic events (e.g. deep
venous thrombosis, pulmonary embolism, myocardial infarction) or of a cerebrovascular
accident.

pectoris).
• Sickle-cell anemia.
• History of migraine with focal neurological symptoms.
• Diabetes mellitus with vascular involvement.
• The presence of a severe or multiple risk factor(s) for venous or arterial thrombosis may also
constitute a contraindication (see under "Special Warnings and Special Precautions for Use".
• Pancreatitis or a history thereof if associated with severe hypertriglyceridemia.
• Presence or history of severe hepatic disease as long as liver function values have not
returned to normal.
• History of deterioration of otosclerosis during pregnancy.

-3-
• Known or suspected sex-steroid influenced malignancies (e.g. of the genital organs or the

• Undiagnosed vaginal bleeding.
• Known or suspected pregnancy.
• Hypersensitivity to the active substances or to any of the excipients.

Pregnancy and lactation

Gynera is not indicated during pregnancy. If pregnancy occurs during treatment with Gynera, further intake should be stopped. However, extensive epidemiological studies have revealed neither an increased risk of birth defects in children born to women who used COCs prior tp pregnancy, nor a tertatogenic effect when COCs were taken inadvertently during early pregnancy. Lactation may be influenced by COCs as they may reduce the quantity and change the composition of breat milk. Therefore, the use pf COCs should generally not be recommended until the nursing mother has completely weaned her child. Small amounts ot the contraceptive steroids and/or their metyabolites may be excreted with the milk but there is no evidence that this adversely affects infant health.

Warnings

If any of the conditions/risk factors mentioned below is present, the benefits of COC use should be weighed against the possible risks for each individual woman and discussed with the woman before she decides to start using it. In the event of aggravation, exacerbation or first appearance of any of these conditions or risk factors, the woman should contact her physician. The physician should then decide on whether COC use should be discontinued.
